PROCEDURE
- Obtain application from nearest eSeva centre
- Pay Rs.100/- in eSeva towards user charges and Rs.60/= service charges
- Pay Rs.500/- in e Seva towards user charges for Passport missing and Rs.60/= towards services
- Fill the application
- Clearly mention date, time and place of loss / missing
- Submit to eSeva Centre
- Will be delivered within 21 day
